We focus on prevention to minimize hospitalizations, including:

• Medication management, insulin shots, etc. through our nursing services
• Nutritional services such as our daily lunch
• Physical, occupational, and speech therapy
• Personal care such as assistance with toileting

Licenses & Affiliation:
Daybreak in Syracuse is licensed under Loretto Health and Rehabilitation Center, Daybreak in Auburn is licensed under Auburn Senior Services, Inc. Both are regulated by the
NYS Department of Health. A member of the Adult Day Health Care Council, and affiliate of the NYS Association of Home and Services for the Aging.

Staff Credentials:
Our team consists of licensed and certified staff providing the best quality of medical services. A registered nurse is on premises at all times of program operation.

Qualification:
Anyone 21 years of age or older requiring assistance with medical service on a weekly basis.

Anyone can contact Daybreak to inquire about enrolling. Eligibility will be determined after an assessment by a registered nurse. Once a physician’s orders are obtained, Daybreak will set up a regular attendance schedule for the individual. Financing options include Medicaid, participating insurance companies and private pay. Our admissions process includes:

• Phone interview to determine type of service needed
• An assessment at our program for our RN to determine needed services and eligibility
• Obtaining orders from the resident’s PCP to complete enrollment
• Continued eligibility by attending a minimum of 1 day/week
