Daybreak’s interdisciplinary care plan team provides integration and coordination between our participants and their personal physicians in order to ensure all aspects of an individual’s needs are addressed.

Anyone can contact Daybreak to inquire about enrolling. Eligibility will be determined after an assessment by a registered nurse. Once a physician’s orders are obtained, Daybreak will set up a regular attendance schedule for the individual. Financing options include Medicaid, participating insurance companies and private pay.
