Adult Day Program
Physical & Occupational Therapy
Our Daybreak Adult Medical Day Program benefits both participants and their caregivers, providing comprehensive health services and care to adults 21 years of age or older, while providing respite to families struggling to care for loved ones. Our interdisciplinary care plan team provides integration and coordination between our participants and their personal physicians in order to ensure all aspects of an individual’s needs are addressed.
PACE CNY provides a wide spectrum of care and services in a potential participant’s home, day health centers, hospitals or long-term facilities. Individuals may receive any number or combination of services as they fit into their personal plan of care developed by the team with the participant. Services are provided or arranged by PACE CNY, a program of the Loretto Family of Care. PACE CNY helps seniors remain in the community with support services.
At Loretto, these are comprised of three private group homes, providing a safe, homelike setting for adults of all ages. These affordable residences provide support and rehabilitation, creating a secure space where individuals can build the skills needed until they are ready to be back out on their own in the community. The goal of the program is to help our residents gently unfold their own lives, encouraging them to advocate for themselves.
