Assisted living programs serve as a bridge between independent living and skilled nursing facilities and provide comprehensive care for seniors, some with complex medical needs. While some assisted living facilities only offer this kind of care to those who can afford it, Loretto ensures beds are available for those who also rely on Medicaid to finance their care.

Loretto offers four assisted living programs across its continuum of care at its Bernardine, Sedgwick Heights, Heritage, and Buckley Landing facilities. All assisted living programs offer customized care plans, including medication assistance, meals, laundry, light housekeeping, and medical oversight.

Sedgwick resident Sue, pictured above, says living at Sedgwick gives her the structure and support she needs to keep living healthy. “They sometimes help me schedule things, like medical appointments and transportation, and they remind me to take my medicine. Other than that, I’m very independent and take care of myself. I like living here and being around people.”

To be eligible for assisted living program care, an individual must complete a financial review, as well as a medical and cognitive screen to determine the appropriate level of care.

Lindzey Nasholts is the Director of Clinical Services for Loretto’s Assisted Living Programs. She says the financial inclusivity of this kind of care for low-income people in Onondaga County sets Loretto’s Assisted Living Program apart.

“There are many assisted living options in our community, but the majority of them are private-pay facilities that either do not accept Medicaid or make available only a certain number of accommodations for the low-income population. At Sedgwick Heights, we are able to provide care for up to 151 individuals; some are private pay while others rely on Medicaid, and all receive the personalized care they need to continue to live independently.”

The registered nurses who manage the individualized care plans for each resident are critical to the success of every Loretto Assisted Living Program. Karen Nolan is one of three registered nurses at Sedgwick Heights, and she said the program’s success is due to the collaborative approach to resident care.

“The Assisted Living Program is successful because of our interdisciplinary team approach to care. As registered nurses, we are responsible for ensuring residents receive the appropriate services and support they need daily.”

Registered nurses work with licensed practical nurses and certified home health aides to help residents with activities of daily living, including dressing, toileting, personal hygiene, and grooming. In addition to personal care and support, Loretto case managers, dining and housekeeping staff, and recreation therapists support each resident’s care plan.

Karen says, “Working together, we make a difference. Our Assisted Living Programs are helping people live as independently as possible, for as long as possible. At Sedgwick Heights, I have seen our residents thrive in our supportive living environment and come together as a community. I am thankful to be a part of that community.”
